Minimum Requirements
- Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
- Active VT Physical Therapist license is required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Must have prior OASIS experience. (Client specific requirement.)
- Must have home health experience, including providing care while traveling from patient to patient. (Client specific requirement.)
- Must have hospice experience. (Client specific requirement.)
- Experience with HealthWyse EMR is preferred.
- Ability to manage changing schedules and adjust caseloads during the day. (Client specific requirement.)
- Facility conducts its own background check. Candidates must be able to comply. (Client specific requirement.)
Assignment Details
- Facility Type: Home Health.
- Shift Name: Days.
- Shift Details: Monday–Friday, 8:00 AM–5:00 PM.
- Shift Pattern: 5x8.
- EMR System: HealthWyse.
- Role requires travel between patient homes for visits and flexibility to adjust schedule during the workday.
- Holiday coverage as needed.
