Minimum Requirements
- Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program recognized by the APTA.
- Active IN state Physical Therapy license is required to start the assignment.
- BLS certification from AHA or ARC may be required.
- Experience providing home health physical therapy preferred.
- Experience with documentation in electronic medical record systems preferred.
- Ability to perform patient evaluations, develop treatment plans, and provide skilled interventions in home settings.
