Minimum Requirements

  • Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program recognized by the APTA.
  • Active IN state Physical Therapy license is required to start the assignment.
  • BLS certification from AHA or ARC may be required.
  • Experience providing home health physical therapy preferred.
  • Experience with documentation in electronic medical record systems preferred.
  • Ability to perform patient evaluations, develop treatment plans, and provide skilled interventions in home settings.
