Minimum Requirements

• Bachelor's, Master's, or Doctoral Degree in Physical Therapy from an accredited program approved by the APTA.
• Active Michigan Physical Therapy license required prior to the start of assignment.
• Current CPR certification.
• Reliable transportation, valid driver’s license, and vehicle insurance.
• Home health experience required.
• Experience with HCHB (Homecare Homebase) EMR required.
• Ability to complete timely documentation, evaluations, and progress notes per agency policy.
• Schedule flexibility to participate in a weekend rotation and potential availability on observed holidays.
• Preferred: Prior experience supervising PTAs or home health aides where allowed by state regulation.

Assignment Details

• Facility Type: Home Health.
• Primary service area includes Wayne County with visits across Oakland, Macomb, Livingston, Genesee, and Washtenaw counties; clinician must be willing to travel within the service area.
• Clinicians are expected to travel to patient homes across the agency service area; scheduler groups visits geographically to limit drive time.
• Must use HCHB EMR for documentation.
• Full Time status.
