Minimum Requirements

  • Bachelor’s, Master’s, or Doctoral degree in Physical Therapy from an accredited program.
  • Current Michigan Physical Therapy license required to start the assignment. Candidates licensed in additional states are encouraged to note multi-state licensure.
  • Current CPR certification required.
  • Home health experience required. Previous work in home health settings preferred.
  • Proficiency with HCHB (Homecare Homebase) electronic medical record required.
  • Valid driver’s license, vehicle insurance, and reliable vehicle required for travel to patient homes.
  • Ability to complete evaluations, develop plans of care, and document per CMS and agency standards.
  • Ability to participate in weekend rotation and potential holiday availability as required by the schedule. This is required by the client.
  • Supervisory experience of home health aides or PTAs preferred, if applicable and allowed by state regulation.

  • Facility type: Home Health.
  • Position type: Travel Physical Therapist, full time.
  • Primary service area: Wayne County with coverage across Macomb, Oakland, Livingston, Genesee, and Washtenaw counties; clinician must be willing to travel within the service area and accept geographically grouped visits to limit driving.
  • EMR requirement: HCHB (Homecare Homebase).
  • Schedule: Requires schedule flexibility including participation in weekend rotation and potential availability on observed holidays.
