Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program recognized by the APTA.
• Active Washington State PT license prior to assignment start.
• Minimum one year of home health physical therapy experience required (client-specific).
• Experience using HomeCare HomeBase (HCHB) is required (client-specific).
• BLS certification from AHA or ARC may be required.
• Ability and willingness to travel within a 50+ mile radius of the office and support patients primarily in Lake Stevens and Snohomish areas (client-specific).
• Productivity expectation: minimum 30 points per week with documentation completed by 10:00 AM the following day (client-specific).

• Shift Details: DAYS: 8HR - M-F.
• Primary patient coverage area: Lake Stevens / Snohomish area with availability to work within a 50+ mile radius of the office.
