Job Title: Physical Therapist (PT)
Job Location:Sacramento, CA
Department:  Rehabilitation Therapy – Skilled Nursing Facility

Position Summary
The Physical Therapist will provide high-quality rehabilitative care to patients within a Skilled Nursing Facility setting. This role focuses on improving mobility, functional ability, and patient independence through individualized treatment plans. The PT will collaborate with the interdisciplinary team to support patient recovery and ensure continuity of care.
Key Responsibilities

• Conduct evaluations and develop individualized treatment plans.
• Provide therapeutic exercises, mobility training, and rehabilitative interventions.
• Monitor patient progress and adjust care plans as needed.
• Document assessments and treatment outcomes accurately.
• Communicate effectively with the care team to ensure coordinated patient care.
• Educate patients and families on treatment goals and home exercises.
• Maintain compliance with facility and state guidelines.

• Active California PT license (must be current).
• Less than 1 year of experience accepted.
• Strong knowledge of rehabilitative practices in SNF settings.
• Ability to work varied shifts.
• Excellent communication and documentation skills.
• Ability to work independently and as part of a care team.
• Strong patient-care and clinical assessment skills.
