Why This Role Matters 

We are seeking a Physical Therapist to provide high-quality rehabilitative care to residents. This role focuses on improving mobility, strength, balance, and functional independence to enhance residents’ safety and overall quality of life. 

Reporting: This position collaborates closely with the rehabilitation team, interdisciplinary staff, and physicians to support comprehensive care planning. 


Role Overview 

  • Evaluate residents and develop individualized physical therapy treatment plans 
  • Implement evidence-based interventions to improve functional outcomes 
  • Document resident progress accurately and in compliance with regulatory requirements 
  • Collaborate with interdisciplinary teams and physicians 
  • Supervise Physical Therapist Assistants and therapy aides, as applicable 
  • Monitor clinical outcomes and contribute to departmental performance 


What We’re Looking For 

  • Active California Physical Therapist license in good standing 
  • Bachelor’s, Master’s, or Doctorate degree in Physical Therapy 
  • New graduates are welcome to apply; experience in skilled nursing or rehabilitation settings preferred 
  • Knowledge of PDPM, Medicare, and skilled nursing facility regulations 
  • Strong leadership, communication, and organizational skills 
  • Commitment to resident-centered, high-quality care
