HOME HEALTH PHYSICAL THERAPIST | DETAILS AND COMPENSATION:

• Location: Jeffersonville IN 47130 – 100% on-site, home health settings
  • Covering : Washington, Scott, and portions of Floyd and Clark counties.
• Payrate: $60.68/hr +$0.46/mile
• Required Availability: Full-Time | Monday – Friday, 8:00 AM – 5:00 PM
  • Call Requirements : Minimum of one weekend per month on call
  • Productivity standard of 30 points.

HOME HEALTH PHYSICAL THERAPIST | SUMMARY AND HIGHLIGHTS:

The Home Health Physical Therapist will be responsible for evaluating and treating patients in their homes, developing personalized treatment plans, and helping patients reach their maximum level of independence

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• Benefits are available to full-time employees after 90 days of employment and include health, optical, dental, life, and short-term disability insurance

• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

HOME HEALTH PHYSICAL THERAPIST | RESPONSIBILITIES:

• Patient Evaluation: Conduct comprehensive physical therapy evaluations and assessments based on physician referrals.
• Care Planning: Establish measurable functional goals and develop customized treatment plans to improve patient mobility and function.
• Clinical Documentation: Maintain and submit accurate evaluation, treatment, and billing records within required timelines.
• Team Leadership: Supervise Physical Therapist Assistants (PTAs) and Aides in accordance with the state practice act.
• Collaboration: Communicate patient progress and needs to the broader treatment team, physicians, and family members.
• Equipment Coordination: Recommend and assist in securing adaptive equipment to enhance the rehabilitation process.

HOME HEALTH PHYSICAL THERAPIST | REQUIREMENTS:

• Education: High School Diploma or GED (verifiable)
• Attendance: Commitment to mandatory attendance during the first 90 days.
• Licensure: Valid and unrestricted Indiana Physical Therapist license.
• Ability to maintain a productivity standard of 30 points.
• Experience with HCHB (Homecare Homebase) and OASIS documentation is highly preferred.
• Strong ability to re-assess treatment effectiveness and modify plans to ensure optimal patient outcomes.
• Adherence to HIPAA, professional practice standards, and company policies.
