Physical Therapist (PT) – Home Health (North County)- $2,500 Sign on Bonus!!!

Location: Escondido, CA (North County San Diego)

Work Type: Full-Time only
Compensation: $55–$65/hr + paid drive time between visits + mileage reimbursement +$2,500 sign-on bonus (Paid drive time included in hourly rate)

A well-established, mission-driven home health provider is hiring an experienced Physical Therapist (PT) to support a primary North County San Diego territory. This role is ideal for PTs who are comfortable working independently in the field, managing a consistent home health caseload, and traveling locally within North County.

What You’ll Do

  • Provide one-on-one physical therapy services in patients’ homes

  • Evaluate patients and develop individualized treatment plans

  • Implement therapeutic exercises to improve mobility, strength, and function

  • Educate patients and caregivers on safety, exercises, and self-care strategies

  • Collaborate with an interdisciplinary care team to deliver patient-centered care

  • Accurately document visits and patient progress within required timelines

  • Manage a consistent home health caseload within assigned territory

What You Need

  • Active California Physical Therapist license

  • Master’s or Doctorate degree in Physical Therapy

  • Minimum 1 year of experience as a Physical Therapist

  • Home health experience strongly preferred

  • New graduates are not eligible

  • Current BLS / hands-on CPR certification

  • Valid driver’s license and reliable, insured vehicle

  • Ability to work full-time with weekend rotation
