About the Role
We are seeking a Pharmacist to provide clinical pharmacy services, medication therapy management, and patient support within a home infusion setting. This role focuses on ensuring safe, effective medication use while collaborating with healthcare teams to improve patient outcomes.
Key Responsibilities
• Provide clinical consultation and drug information to healthcare providers regarding therapy, interactions, and contraindications
• Oversee medication dispensing, storage, and inventory management
• Support patient care through education, monitoring, and therapy management
• Collaborate with providers, insurance companies, and care teams to ensure accurate and compliant pharmacy services
• Assist with workflow optimization, audits, and quality improvement initiatives
• Contribute to team development and pharmacy operations
Required Qualifications
• PharmD or Bachelor’s degree in Pharmacy
• Active Pharmacist license in South Carolina (in good standing)
• Basic computer skills and strong attention to detail
• Strong knowledge of medical terminology and clinical pharmacy practices
Highly Preferred
• 1+ year of experience in home infusion or hospital pharmacy setting
• Experience with Epic or similar pharmacy systems

• Ability to collaborate with interdisciplinary healthcare teams
• Excellent organization and attention to detail
• Strong communication and patient education skills
