Qualifications:
- Valid RN licensure in the state of New Hampshire
- BLS (AHA) certification required
- Proven experience in clinical coordination and patient care preferred
- Ability to work independently and collaboratively within a healthcare team
- Strong organizational, communication, and documentation skills
Key Responsibilities:
- Clinical Coordination: Oversee and support clinical staff to ensure efficient patient flow and high-quality care delivery. Coordinate daily clinical activities, staffing, and support to optimize clinic operations.
- Patient Care Delivery: Provide direct, comprehensive nursing care to patients, including assessment, planning, implementation, and evaluation of nursing interventions for optimal health outcomes.
- Patient Management: Function as an Office Nurse by triaging patient calls, managing medication refills, and coordinating outpatient patient care to ensure seamless service delivery.
- Collaboration: Work closely with physicians, healthcare professionals, and support staff to develop personalized care plans and address patient needs effectively.
- Documentation & Reporting: Maintain accurate and detailed medical records, ensuring compliance with all regulatory standards. Report patient condition changes and any concerns promptly to relevant team members.
- Medication Management: Administer prescribed medications and treatments following established protocols and safety guidelines.
- Patient Education & Advocacy: Educate patients and their families about diagnoses, treatment options, and preventive care practices. Promote patient rights and provide compassionate support throughout their care journey.
- Regulatory Compliance & Continuous Improvement: Adhere to hospital policies, procedures, and industry standards. Engage in ongoing professional development to stay current with medical advances and best practices.
- Additional Duties: Perform other clinical or administrative tasks as assigned to support practice excellence.
