The Role
As a Pediatric RN Case Manager in Brooklyn, NY, you will provide one-on-one, home-based nursing care primarily to well-babies (ages 0–2). This is a field-based role that eliminates office "desk time," allowing you to focus entirely on assessments, parent education, and care coordination.
What You’ll Do
Holistic Assessments: Perform initial assessments and recertifications for pediatric cases within your assigned Brooklyn territory.
Clinical Strategy: Develop and implement individualized care plans tailored to the unique needs of infants and their caregivers.
Empowerment: Provide vital clinical care and parent/caregiver education to ensure the best outcomes for our youngest patients.
Collaboration: Act as the lead coordinator for an interdisciplinary care team, ensuring all services are aligned.
Documentation: Use streamlined electronic systems to maintain accurate and timely patient records from the field.
What You Bring
Licensure: Active New York State Registered Nurse (RN) license.
Experience: Strong background in pediatric inpatient care, NICU, PICU, or pediatric clinical settings.
Soft Skills: Exceptional assessment and communication skills with the ability to work independently in a community setting.
Commitment: A passion for newborn care and supporting families during the critical early stages of life.
