About the Role
The Patient Care Manager will lead care coordination efforts, support patient engagement, and ensure the delivery of high-quality, patient-centered services across virtual care platforms. This role serves as a key liaison between patients, healthcare providers, and internal teams, ensuring seamless communication and effective care management.

Key Responsibilities
Oversee daily patient care coordination and remote support services
Supervise, mentor, and support patient care coordinators and support staff
Ensure timely patient follow-ups, care updates, and service assistance
Monitor patient cases, escalations, and service quality standards
Collaborate with healthcare providers to align care plans and patient needs
Maintain accurate patient records and ensure HIPAA compliance
Analyze patient care metrics and implement process improvements
Train staff on patient communication best practices and care protocols
Address complex patient concerns with professionalism and empathy
Required Qualifications
Bachelor’s degree in Healthcare Administration, Nursing, Public Health, or a related field
Minimum of 1 year of experience in patient care coordination, healthcare operations, or case management
Prior leadership or team management experience
Strong understanding of healthcare workflows and patient service standards
Excellent communication, organizational, and problem-solving skills
Experience with EMR/EHR systems and remote care tools
Ability to manage multiple patient cases while maintaining high-quality service
Preferred Qualifications
Nursing background (RN/LPN) or clinical experience
Experience in tele health or remote healthcare environments
Knowledge of insurance processes and care authorizations
