Job Title: Patient Care Coordinator – Critical Need
Location: Salem, OR 97301
Duration: 2 Months+ (Possible Extension) Contract

Shift: 08:00 AM – 06:30 PM (Days)

Minimum Requirements:

· Active RN Oregon License & BLS

· 1 year of current experience as RN Case Manager (Acute/Hospital)

· Skills: Coordinating patient care, scheduling, or supporting clinical teams preferred

Summary:

Supports patient flow, communication, and coordination activities to ensure timely, accurate, and high‑quality care delivery.

Responsibilities:

·         Coordinate patient scheduling, messages, records, and follow‑up activities to support efficient care delivery.

·         Communicate with patients, providers, and staff to ensure accurate information exchange and smooth transitions.

·         Maintain documentation, assist with regulatory compliance needs, and support daily operational workflows.
