Job Title: Ontario County DSS Case Manager
Location: Farmington
Schedule: Monday - Friday, 8:30a-4p.
Supervisor: Program Manager
Classification: Full Time
Department : Outpatient Clinic
Minimum Qualifications: Associates Degree in Health/Human Services. Valid NYS Drivers’ License. Bachelors Degree preferred. Pursuit of QHP is encouraged.
Job Summary: In accordance with the organization’s mission, vision and values, the Wayne County DSS Case Manager is responsible for case management for individuals in need of addictions services.
