What You’ll Do:
- Provide occupational therapy services to residents and initiates referrals when appropriate and in accordance with Rehab Department Procedures.
- Conduct screenings of residents at regular intervals to determine need for intervention/treatment.
- Evaluate residents to obtain data necessary for treatment planning and implementation and conducts specialized evaluations as indicated.
- Interpret and communicate evaluation findings to residents, families and team members.
- Establish individual resident schedules in conjunction with other team members
- Develop treatment plans by identifying a target list, including short term and long-term goals and methods to achieve identified goals.
- Coordinate treatment plan with resident, family and other team members.
- Implement and conduct treatment as outlined in treatment plan.
- Monitor resident’s response to intervention and modify treatment as indicated to attain goals in accordance with physician orders.
- Develop appropriate home or programming within the community to maintain and enhance the performance of the resident in their own environment.

Qualifications
- Current Washington Occupational Therapist license
- Current BLS Card
- Strong clinical skills and a compassionate approach
- Dependable and efficient with excellent teamwork
- Previous long-term care or rehab experience a plus
