| SETTING: | SNF/LTC - Conduit that connects individuals from the hospital to home, at the pace that is appropriate for them. Initially, this partnership utilized 31 beds at Providence Pacific Campus. In November 2022 it expanded to a second floor, for a total of 62 beds dedicated to serve this population. |
