Nurse Practitioner/Physician Assistant - Primary Care

We are seeking a Nurse Practitioner or Physician Assistant specializing in Primary Care to join their growing clinical team in Brooklyn. This is a full-time, permanent opportunity to make a meaningful impact while delivering exceptional care to both long-term and short-term residents in a collaborative healthcare environment.

What You’ll Do
In this role, you will serve as a primary care provider, delivering comprehensive patient care through physical examinations, medical history reviews, and ongoing health assessments. You’ll work closely with physicians, nursing staff, and the broader care team to ensure patients receive the highest standard of coordinated, evidence-based care.

A key part of the role involves building strong relationships with residents and their families, educating them on treatment plans, and creating personalized care strategies that align with each patient’s needs, goals, and preferences. You will also perform therapeutic procedures such as administering injections and immunizations, suturing, and managing wounds and infections.

Beyond direct patient care, you’ll help drive clinical excellence by maintaining accurate documentation, ensuring regulatory compliance with federal and state healthcare requirements, and contributing valuable insights to the care team on both short-term and long-term patient management strategies. You’ll also promote wellness through patient education, counseling, and preventative health initiatives.

• Board Certified Nurse Practitioner or Physician Assistant

• Strong clinical skills and attention to detail

• Excellent bedside manner and commitment to patient-centered care

• Ability to collaborate effectively within a multidisciplinary medical team

• Experience with health promotion, medication administration, pain management, and infection control

• Hospital experience is welcome and will be considered
