You’ll work closely with a registered nurse and collaborate with each patient’s primary care provider to identify red flags, manage chronic conditions, and keep patients healthy at home. Patient Support & Follow‑Up

Build strong rapport through compassionate bedside manner
Conduct regular follow‑up visits to evaluate progress and adjust care plans
Maintain accurate EMR documentation within 48 hours of each visit
Collaboration & Quality
Work closely with physicians, RNs, and other care team members
Participate in quality improvement initiatives
Adhere to all regulatory, compliance, and privacy standards

Participate in paid telephonic on‑call rotation every 6–8 weeks
Provide 24/7 support to patients during on‑call weeks
