Position Description 

Penn Medicine Home Infusion Therapy program is hiring for a full-time Nurse Practitioner! This position will be responsible for the nutritional care of Penn Home Infusion Therapy patients. The Nurse Practitioner will function as a professional consultant to the Medical Team to assure cost-effective, high-quality, and current nutrition/nutrition support practices (parenteral and enteral infusion/access).

In this position, the Nurse Practitioner will:

  • Perform nutrition assessments and provide education on nutrition access/infusion
  • Develop specialized nutrition care plans and perform ongoing monitoring of infusion therapy
  • Assist in coordination of the discharge planning of patients requiring home infusion therapy
  • Maintain comprehensive knowledge base in nutrition support (infusion/access) therapy
  • Evaluate, interpret, and apply results of research into nutrition support practices and delivery of patient care services

Minimum Requirements 

Nurse Practitioner candidates must be licensed to practice professional nursing in the Commonwealth of Pennsylvania, and possess a MSN or DNP and certification as an Adult Gerontology Primary Care Nurse Practitioner or Family Nurse Practitioner.
