We are seeking a compassionate Nurse Practitioner or Physician Assistant to join Hill Country Mission for Health (HCMH), a nonprofit clinic providing primary care to underserved adults in our community. This is an opportunity to make a real difference by helping patients manage chronic conditions, navigate healthcare, and improve their overall health.
Essential Functions:
- Provide primary care, including assessment, diagnosis, and treatment of common chronic and acute health conditions.
- Manage patient care using limited resources, including labs, medications, and referrals.
- Refer patients for specialty care, mental health, women’s health, or emergency services as needed.
- Educate patients and families to support informed health decisions.
- Document care accurately in our electronic medical record system.
- Participate in staff meetings, community outreach, and clinic events.
What We’re Looking For:
- Current Texas license as a Nurse Practitioner, Physician Assistant, or Physician.
- BLS certification required (ACLS preferred).
- Strong computer skills; EMR experience helpful.
- Compassion, professionalism, and cultural sensitivity.
- Ability to work independently in a small clinic setting.
Preferred:
- Bilingual in English and Spanish.
- Experience serving underserved or uninsured populations.
