A healthcare provider specializing in telehealth services is seeking a skilled Nurse Practitioner (NP) to join their telehealth team. The ideal candidate will conduct primary visits remotely, provide assessments, and collaborate with assisted living facility staff to ensure comprehensive care. 
Responsibilities: 
Conduct primary care visits via telehealth for residents in assisted living facilities. 
Evaluate, diagnose, and manage patient care in accordance with clinical guidelines. 
Communicate effectively with facility staff, patients, and families. 
Maintain accurate and timely documentation in electronic medical records. 
Collaborate with other healthcare providers to coordinate patient care. 
Adhere to telehealth protocols and compliance regulations. 
Qualifications: 
Licensed Nurse Practitioner (NP). 
Experience in primary care, geriatrics, or telehealth preferred. 
Excellent clinical assessment, diagnostic, and communication skills. 
Comfortable working in a telehealth environment and using electronic health records. 
Professional, compassionate, and detail-oriented.
