Job Details
Deliver care within senior living and post-acute communities
Diagnose and manage acute, episodic, and chronic conditions
Order and interpret diagnostic testing
Prescribe medications as appropriate
Develop and manage patient care plans
Collaborate with interdisciplinary care teams
Maintain accurate, timely documentation
Educate patients and caregivers on disease management
Requirements:
Active Illinois NP or PA license
Minimum 6 months of NP/PA experience
Reliable, insured transportation
Strong clinical judgment and independent decision-making skills
Preferred:
2+ years in long-term care or skilled nursing
Experience managing cardiac or complex chronic conditions
