As a Nurse Practitioner (NP) at CenterWell Senior Primary Care, you will be part of a collaborative, multidisciplinary team that includes physicians, pharmacists, care coaches, behavioral health specialists, and more—all working together to deliver personalized, high-quality care.

You will work onsite at our Chesapeake clinic in Chesapeake, VA. You will report to our Associate Medical Director for Tidewater Region.

Your Role & Responsibilities

  • You will provide comprehensive care to seniors following clinical standards.
  • You will collaborate daily with your care team to ensure coordinated, high-quality care.
  • You will manage referrals, hospital/SNF coordination, DME, and home health services.
  • You will document care accurately and efficiently with support from quality-based coders.
  • You will participate in on-call rotation and support growth plans at your center.
  • You will have passion for providing value-based care.

What We're Looking For

Required:

  • Master's Degree in Nursing with board certification.
  • Active, unrestricted NP license in the state of practice.
  • If selected for this role, you will be required to be screened for TB

Preferred:

  • Experience in value-based care, primary care or Medicare Advantage settings.
  • Active DEA license and Medicare/Medicaid provider numbers or ability to obtain DEA license and/or Medicaid and Medicare numbers
  • Familiarity with HEDIS quality indicators and Medicare guidelines.

Additional Information:

  • Language Proficiency Testing: any associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency. Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government.

Pay Range

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.


 

$115,200 - $158,400 per year


 

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
