A-Line Staffing is now hiring a Full-Time, Field Based, Nurse Case Manager – Long-Term Care, in Jacksonville, NC! (COVERING JACKSONVILLE, WILMINGTON, & NASH COUNTIES). (TRAVEL 1 WAY CAN BE UP TO 2.5-2.75 HOURS AWAY. WOULD BE YOUR ONLY APPOINTMENT).

About the Role:
We are seeking an experienced Nurse Care Managers to provide holistic care management for members with Long Term Support/Services (LTSS) needs. This is a member-facing, field-based role requiring both remote administrative work and in-person member visits.

You will assist in developing personalized care plans, monitor service delivery, and educate members, caregivers, and providers on available services and benefits. This role involves collaboration with discharge planners, physicians, and other healthcare professionals to ensure seamless coordination of care.

Nurse Case Manager – Long-Term Care Compensation

  • The pay for this position is $40/hr + mileage reimbursement
  • Benefits are available to full-time employees after 90 days of employment

Nurse Case Manager – Long-Term Care Highlights

  • This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
  • The required availability for this position is Monday–Friday with rotating shifts (8:00 AM–5:00 PM, 8:30 AM–5:30 PM, or 9:00 AM–6:00 PM) plus required evening and weekend on-call coverage. Overtime available as needed.

Nurse Case Manager – Long-Term Care Responsibilities

·         Manage a caseload of LTSS members; conduct monthly and quarterly contacts.

·         Perform member assessments and document care plans.

·         Coordinate and authorize referrals for services.

·         Monitor service delivery, identify gaps, and ensure quality care.

·         Collaborate with healthcare providers and discharge planners to develop care plans.

·         Assist members with complaints or appeals as needed.

·         Travel 80% of the time; mileage and lodging reimbursed.

Nurse Case Manager – Long-Term Care Requirements

Required:

  • Bachelor’s degree
  • Licensed Clinical Social Worker (LCSW/LCSW-A) and/or Registered Nurse (RN/BSN/ADN)
  • 2+ years physical health care management experience (hospital CM, home health, discharge planning, or long-term care)
  • Valid driver’s license
  • Tech-savvy; proficient in Microsoft Office and electronic medical records

Preferred:

  • 4–6+ years physical health care management experience
  • Home health, community health, or member-facing experience
  • Experience in discharge planning and utilization management
