The Role:
You’ll serve as the clinical guide for individuals receiving care in the community. Your work will directly impact quality of life, independence, and health outcomes.
- Complete intake assessments and coordinate care services
- Develop and manage individualized care plans
- Monitor health data and adjust care as needed
- Support and guide caregivers and staff
- Conduct supervisory visits and provide education to families
- Address concerns and ensure high-quality service delivery
- Collaborate with physicians and interdisciplinary teams
Requirements:
- Active Missouri RN license
- At least 2 years of nursing experience
- Strong clinical judgment and attention to detail
- Confidence working independently while staying organized
- A genuine passion for helping others live well
