Multi-Site MDS Coordinator (RN) – Remote/Hybrid

Location: Eastern Massachusetts
Schedule: Full-Time | Primarily Remote with Travel 1-2 Days per Week
Compensation: $115,000 Annual Salary

We are seeking an experienced MDS Coordinator (RN) to provide regional coverage and remote clinical support across skilled nursing facilities in Eastern Massachusetts. This role supports facilities during MDS vacancies, leaves, and transitional periods.

This position is primarily remote with required on-site travel 1-2 days per week.

Responsibilities:

• Complete and submit MDS assessments and resident interviews
• Develop and update care plans; attend care plan and clinical meetings
• Review assessments for accuracy and compliance
• Support PDPM, CMI, QM, and QRP processes
• Communicate effectively with interdisciplinary teams
• Travel to assigned facilities as needed (1-2 days/week)

Qualifications

• Active RN license in Massachusetts (or ability to obtain)
• Minimum 2 years MDS experience required
• Skilled nursing / long-term care experience required
• Strong attention to detail, organization, and communication skills
• Computer savvy and comfortable working remotely
• Team-oriented and adaptable to multiple facility environments
