What You'll Be Doing

As a Home Health MSW, you'll visit patients in their homes to provide emotional support, counseling, and practical help. You'll work closely with nurses, therapists, and doctors to make sure patients have the resources they need to live safely and comfortably.

Day-to-day, you'll:

• Visit patients and assess their social, emotional, and practical needs

• Provide short-term counseling, crisis intervention, and support for patients and caregivers

• Connect patients with community resources (housing, food, transportation, financial support, etc.)

• Collaborate with the care team to create personalized care plans

• Support patients and families through serious illness, disability, or life transitions

• Keep clear, timely documentation using Home Care Home Base (HCHB)
• Who We're Looking For

• Master's degree in social work (MSW) required

• Licensed to practice in [Insert State] (LCSW preferred, but not required)

• Experience in home health or healthcare is a big plus

• Great listener, team player, and problem-solver

• Comfortable working independently and traveling to patients' homes

• Familiar with Home Care Home Base (HCHB) or willing to learn
