Job Summary:
• Join our team as a Medical Case Manager in a supportive housing setting located in the Brooklyn, NY.
• This contract position offers flexible weekday scheduling and requires a Registered Nurse to provide care and support for individuals with psychiatric and behavioral health needs.
Requirements:
• NYS Registered Nurse License
• 1-2 years of experience in behavioral and mental health
• Experience in case management
• Strong communication skills
• Excellent organizational skills
• Ability to maintain confidentiality
• Interpersonal skills
Qualifications:
• Registered Nurse licensed in New York State
• 1-2 years of experience with behavioral health population
• Experience in medical case management
• Strong assessment and intervention skills
• Proficient in developing and implementing care plans
• Ability to interact with healthcare providers effectively
• Experience in crisis intervention
