Experience/Qualifications
Licensed Vocational Nurse (LVN) certification in good standing.
2 years Clinic Experience
Proven experience in care coordination within a healthcare setting.
Commitment to continuous professional development and learning.
Strong communication skills to effectively collaborate with healthcare teams.
Ability to analyze patient data and adjust care plans accordingly.
Ability to work independently while being accountable for results.
Knowledge of healthcare regulations and compliance standards.
Proficiency in electronic health record (EHR) systems.
Strong organizational skills to manage multiple patient care plans.
Capability to educate patients and families on health management.
Responsibilities
Coordinate patient care plans and ensure effective communication among healthcare providers.
Monitor patient progress and adjust care plans as necessary to achieve health goals.
Maintain accurate and up-to-date patient records in compliance with healthcare regulations.
Collaborate with multidisciplinary teams to optimize patient care and outcomes.
Ensure compliance with all healthcare policies, procedures, and standards.
Educate patients and families on care plans and health management strategies.
Identify and address potential barriers to patient care and propose solutions.
Develop and maintain strong relationships with patients and their families.
Participate in continuous professional development to enhance skills and knowledge.
Utilize data and metrics to assess care effectiveness and improve service delivery.
Report on care outcomes and provide feedback to improve patient services.
Contribute to the development of care protocols and best practices.
