Job Title: Licensed Practical Nurse (LPN) – PRN
Location: Flagstaff, AZ 86004
Schedule: 12-hour shifts (Day or Night)

  • Day Shift: 6:00 AM – 6:30 PM
  • Night Shift: 6:30 PM – 6:00 AM
    Employment Type: PRN (As Needed – No Guaranteed Hours)

Job Summary

"Navitas Healthcare, LLC" is seeking a dedicated and compassionate Licensed Practical Nurse (LPN) to join our team on a PRN basis in Flagstaff, AZ. The ideal candidate will provide high-quality nursing care in a long-term care or skilled nursing setting, ensuring the health, comfort, and well-being of residents while collaborating with interdisciplinary teams.


Key Responsibilities

  • Administer medications and treatments within scope of practice
  • Deliver direct patient care, including wound care, tracheostomy care, catheter care, and other clinical procedures
  • Monitor patient conditions and report responses to medications and treatments
  • Perform initial nursing assessments for newly admitted residents
  • Assist in care planning, including MDS assessments and ongoing evaluations
  • Maintain accurate, timely, and compliant clinical documentation
  • Communicate effectively with residents, families, and healthcare team members
  • Participate in shift reports to ensure continuity of care
  • Ensure availability of necessary supplies and medications
  • Support Quality Assurance and Performance Improvement (QAPI) initiatives
  • Attend staff meetings and contribute to a collaborative team environment
  • Respond promptly and effectively to medical emergencies
  • Maintain a safe, clean, and organized patient care environment

Qualifications

  • Active Licensed Practical Nurse (LPN) license in the state of Arizona (required)
  • Minimum of 1 year of nursing experience (long-term care or geriatric experience preferred)
  • Strong knowledge of geriatric nursing practices and patient care standards
  • Proficiency in medication administration and clinical procedures
  • Excellent communication, organizational, and critical-thinking skills
  • Ability to work independently and as part of a multidisciplinary team
