Position Overview:
We are currently seeking a Licensed Practical Nurse (LPN) is a licensed healthcare practitioner who provides direct nursing care to individuals under the supervision of a Registered Nurse, physician, or other licensed healthcare provider. The LPN delivers patient-centered care in accordance with established treatment plans, applicable laws, and professional standards, including Article 139 of the Education Law. Responsibilities include administering medications, monitoring patient conditions, supporting activities of daily living, maintaining clinical documentation, and providing health education and emotional support.

Qualifications:
- Education: Graduation from an accredited practical nursing program.
- Licensure:
- Current licensure and registration as a Licensed Practical Nurse (LPN) in the applicable state.
- Eligibility for appointment under open competitive standards at the time of hire.

Key Responsibilities:
- Job responsibilities:
- Provide direct nursing care consistent with individualized treatment plans and established procedures.
- Administer prescribed medications and treatments.
- Assist Registered Nurses, physicians, and other licensed providers in identifying patient needs and implementing care plans.
- Observe, monitor, and document patient symptoms, responses to treatment, and changes in condition.
- Prepare patients for examinations and assist during treatments.
- Maintain accurate medical charts and clinical documentation.
- Provide emotional support and health education to individuals and, when appropriate, their families.
- Support a safe, clean, and therapeutic care environment.
- Monitor supplies and complete requisitions as needed.
- Supervise and observe unlicensed direct care staff, as assigned, and report findings to supervisory staff.
- Physical Examination Responsibilities:
- Measure and record vital signs, including temperature, blood pressure, pulse, and respiration.
- Observe physical and behavioral indicators of health status.
- Assist with physical examinations and clinical procedures as directed.
- Medical Oversight Responsibilities:
- Carry out nursing tasks under the supervision of a Registered Nurse, physician, or licensed healthcare provider.
- Communicate observations, patient responses, and clinical concerns promptly, supervising medical staff.
- Ensure treatments and medications are administered according to prescribed orders and standards of care.
