Requirements:
• Licensed LPN in the state of New York
• Proficiency in word processing and spreadsheet software
• Strong verbal and written communication skills
• Interpersonal skills for effective interaction with participants
• Ability to conduct health assessments and manage referrals
• Experience in administering medications and injections
• Capability to provide health management education
Qualifications:
• LPN license in New York
• Experience in conducting physical health assessments
• Skills in managing and administering medications
• Experience in coordinating with medical providers
• Ability to educate participants on health management
• Experience in supporting individuals with mental illness
• Proficient in using general technology tools
