What You’ll Do:
As a Licensed Nurse Care Coordinator, you will coordinate patient care, ensure adherence to care plans, and facilitate communication between patients, caregivers, and healthcare providers. Your expertise will support clinical management and improve healthcare delivery through telephonic care and care gap analysis. Your role is essential in driving quality improvements and fostering strong relationships with patients and healthcare teams.

Required Skills:
  • Valid Texas Nursing License (LVN) / Practical Nurse license with Nursys license verification
  • Minimum of 3 years clinical or home health experience
  • Knowledge of HEDIS, CMS guidelines, or care gap analysis
  • Experience in primary care settings and telephonic care coordination
  • Proficiency in Epic EMR and Microsoft Office, especially Excel
  • Excellent telephonic communication and customer service skills
  • Ability to work effectively with elder populations in a compassionate manner

Nice to Have Skills:
  • Experience with care coordination tools and population health management software
  • Knowledge of Medicare, Medicaid, or other private insurance protocols
  • Bilingual abilities to serve diverse patient needs

Preferred Education and Experience:
  • High School Diploma or equivalent (submit proof of education)
  • At least three years of clinical or home health care experience in a related setting

Other Requirements:
  • Resident of Texas with the ability to work onsite only
  • Must be able to attend orientation (billable at 70% of the hourly rate for the first 36 hours)
  • Availability Monday through Friday, with flexibility for additional hours as needed
  • Willingness to work with elder populations in a clinical or community-based setting
  • Valid licensure verification through Nursys portal is required upon submission
