Job Title: Licensed Independent Social Worker (LISW)
Job Type: Full-Time 

We are seeking a Licensed Independent Social Worker (LISW) to join our team. The LISW will play a pivotal role in providing holistic care to our patients, with a focus on pain management, addiction medicine, and behavioral health. This position offers both full-time and part-time options, with the opportunity for some remote work to provide telehealth services.

Qualifications:
  • Current licensure as a Licensed Independent Social Worker (LISW) in South Carolina.
  • Master's degree in Social Work (MSW) from an accredited institution.
  • Strong knowledge and experience in pain management, addiction treatment, and behavioral health.
  • Excellent interpersonal and communication skills.
  • Ability to work independently and as part of a multidisciplinary team.
  • Empathy and sensitivity when working with patients facing complex challenges.

Responsibilities:

Assessment and Treatment Planning:
  • Conduct comprehensive assessments of patients' physical, emotional, and social needs.
  • Develop individualized treatment plans that address pain management, addiction medicine, and behavioral health concerns.
Therapeutic Interventions:
  • Provide evidence-based therapeutic interventions to support patients in managing pain, overcoming addiction, and improving their mental health.
  • Conduct individual and group therapy sessions.
Patient Education:
  • Educate patients and their families on pain management strategies, addiction recovery tools, and behavioral health resources.
  • Promote self-care and healthy coping mechanisms.
Case Management:
  • Collaborate with healthcare providers and external agencies to coordinate care and ensure a holistic approach to treatment.
  • Monitor and update treatment plans as needed.
Documentation:
  • Maintain accurate and up-to-date patient records, treatment plans, and progress notes.
  • Comply with all legal and ethical standards for documentation.
