Key Responsibilities:

• Develop and implement grief/bereavement work with families.
• Facilitate and coordinate the continuum of patient care to include scheduling of treatments, associated diagnostic testing, surgery, and follow-up outpatient visits.
• Effectively utilize health care resources to optimize patient outcomes
• Provide counseling as needed to address each stage of diagnoses and treatment.
• Participate in the monthly Tumor Board meetings.
• Conduct comprehensive assessments, including history-taking to obtain information about barriers to care, home environment, family relationships, and patient’s personality traits.
• Collaborate closely with healthcare team to ensure coordinated and comprehensive patient care.
• Document patient/family encounters accurately and timely in the electronic health record system.

Education Requirements:

• LCSW with experience
