Schedule: Full Time, Day 

Salary: $49,140.00 - $71,867.25 per year

• Advocate for residents’ rights, dignity, and self-determination by actively listening to their concerns, addressing psychosocial needs, and ensuring their preferences guide all care planning and decision-making.

• Conduct comprehensive biopsychosocial assessments to identify each resident’s emotional, social, cultural, and environmental needs, developing individualized care plans that promote safety, comfort, and quality of life.

• Provide ongoing counseling, emotional support, and crisis intervention to residents and their families, helping them navigate adjustment to long-term care, changes in health status, and end-of-life planning with compassion and respect.

Licensure / Certification / Registration:

• Master Social Worker, preferred. 

Education:

• High school diploma/GED with 2 years of experience, or Associate's degree, or Bachelor's degree required.

Work Experience:

• 3 years of experience preferred.
