Pennsylvania Hospital is seeking a Licensed Clinical Social Worker to join our Cancer Program Team. This is a full time, day shift opportunity.

Summary:

  • The Licensed Clinical Social Worker provides psychosocial assessment, case management and individual counseling to patients and their families within the Abramson Cancer Center at Pennsylvania Hospital, as part of a social work team.

Responsibilities:

  • Provides psychosocial assessment and individual therapy to patients/family members on a short term basis (50% of role)

· Provides oncology case management, (50% of role) which includes:

    • Coordinating care and services across internal/external providers
    • Connecting patients with community resources
    • Collaborating with families, caregivers, and support systems as appropriate
    • Facilitating referrals and follow-up to promote continuity of care
  • Provide support for cancer program staff through facilitation of process groups and educational sessions.
  • Provides consultation and training on psychosocial aspects of human behavior and complex environments including distress management.

Credentials:

  • Licensed Clinical SW (Required)

Education or Equivalent Experience:

  • Master of Arts or Science (Required)
  • Education Specialization: Social Work Equivalent Experience: •And 2+ years Health Care experience counseling and/or employment in social work in a healthcare setting.
  • Oncology experience preferred
