OVERVIEW
This is a direct care position and Lead Direct Support Professionals are
counted in staff ratio. Provides oversight, scheduling, direction,
monitoring, and coordination of program activities of assigned
community-based residences for individuals with developmental
disabilities. Assists with the development and review of Individual
Service Plans (ISPs), behavior plans, and medication plans. Monitors and
maintains client case records ensuring necessary guidelines and
standards are followed. Monitors direct care staff and ensures site and
equipment are properly maintained. Assists with the development of staff
schedules and ensures required documentation is properly completed and
filed. Responsible for assuring staff and client coverage to meet
contractual obligations, quality care for residents, compliance with all
CCS and Community Living Program (CLP) policies, and assuring licensing
standards are met.Position may from time to time be stressful and
require a high demand of performance.May perform other duties as
assigned.
ESSENTIAL FUNCTIONS
- Assists in all aspects of assigned residential settings
- Responsible for assuring that client records are maintained to meet
guidelines of Department of Economic Security (DES) and Division of
Developmental Disabilities (DDD) program monitoring, contract
compliance, and agency policies and procedures
- Ensures all program operations are conducted in safe and appropriate
manners, and reports issues to the Site Supervisor and Program
Manager
- Under the direction of the Program Manager, maintains financial
accountability of program household and client account monies
following identified policies and procedures for all tasks and
processes
- Maintains a flexible working schedule based upon program need
- Participates as an active member of the individual ISP teams,
maintaining professional and courteous contacts with parents,
guardians, family members, day program representatives, and medical
and therapy personnel
- Provides direct care at the program sites as directed or required to
meet program needs
- Responsible for on-call duty in case of emergency; shares on-call
responsibility with other Lead DSPs
- Works as a member of the community Living Program Management Team to
promote the agency's policies, procedures, and philosophy to
co-workers, DDD agents, other providers, and the community at large
- Ensures quality indicators are present in the program on a continual
basis
- Communicates regularly with the CLP Administration on staffing
issues, resident concerns, safety issues, timecard issues, and
established procedural changes
- Takes residents to medical appointments, case planning meetings, and
medical reviews as needed based on medical advocate schedule
- Responsible for daily checklist of cleaning duties
- Picks-up medication and documents in Medication Administration
