Description:

The Judge Group is looking for a nurse practitioner or physician assistant to perform in home health assessments in Chautauqua County, NY with one of the largest healthcare providers nationwide. 


Location: Chautauqua County, NY

Employment Type: Per Diem / Contract

Duration: On-going as needed, depending on needs in the area.

Compensation: $100 to $110 per assessment.

Schedule/Shift: Days and evenings; flexible scheduling with 60–75 minute appointments

 

Overview

Our client is seeking a Per Diem Nurse Practitioner or Physician Assistant to conduct in-person annual preventive health risk assessments for members enrolled in Medicare, Medicaid, and Affordable Care Act health plans. Assessments are completed at no cost to the member and take place within the individual’s personal living space, following health plan and quality expectations.

This role is ideal for providers with primary care and/or in-home health experience who enjoy flexible scheduling, autonomous field work, and delivering high-quality preventative care in a community setting.

 

Key Responsibilities

  • Perform in-person annual preventive health risk assessments in members’ homes
  • Complete a minimum of 15 appointment visits per month
  • Ensure assessments meet quality standards, health plan expectations, and member satisfaction
  • Document and submit complete chart notes within 24 hours of each visit
  • Respond to charting queries within 48 hours
  • Maintain clear and timely communication with clinical leadership
  • Participate in required self-paced and instructor-led training within designated timeframes
  • Attend periodic trainings and meetings related to the role
  • Enter personal living spaces comfortably and professionally
  • Comply with all HIPAA and confidentiality requirements

Required Qualifications

  • Master’s or Doctorate degree from an accredited Nurse Practitioner program (FNP, AGNP, ANP) or
    Master’s degree from an accredited Physician Assistant program
  • Home health experience within the last 5 years
  • Strong computer skills with proficiency in electronic medical records
  • Understanding of CMS and ACA risk adjustment models
  • Ability to accurately and thoroughly document clinical findings
  • Effective verbal and written communication skills
  • Ability to make critical decisions while following policies and procedures
  • Ability to adapt to changing conditions and responsibilities
  • Willingness to travel within a 50-minute radius of assigned service areas

 

Preferred Qualifications

  • Primary care provider (PCP) experience
  • At least 1 year of in-home assessment experience

 

Certifications / Licensure

  • Active, unencumbered NP or PA license in the state of practice
  • Current RN license (for Nurse Practitioners)
  • National board certification (ANCC, AANP, or NCCPA)
  • Active Medicaid ID number for the state
  • Active Medicare PTAN number
  • Current BLS certification
  • Valid state driver’s license
  • Proof of valid automobile insurance appropriate for the state of practice
