Overview
The Hybrid Healthcare Consultant/Case Management Coordinator offers a flexible, autonomous schedule while supporting members through both telephonic and face-to-face interactions. In this position, you will manage a diverse caseload, including medically complex and high-need members, with a strong focus on care coordination, wellness improvement, and long-term health outcomes.
Key Responsibilities
- Coordinate case management activities for Medicaid Long Term Care/Comprehensive Program members
- Conduct comprehensive assessments using care management tools and data review
- Develop, implement, and monitor individualized care plans
- Collaborate with PCPs, specialists, and multidisciplinary teams to ensure optimal care
- Facilitate services such as prior authorizations, medication reviews, and community resource support
- Identify and escalate quality of care concerns through appropriate channels
- Educate and empower members to make informed healthcare decisions
- Monitor, evaluate, and document care plans
- Conduct in-person visits (homes, assisted living, nursing facilities) and telephonic outreach
Required Skills & Qualifications
- Fluent in English and Spanish (REQUIRED)
- Must reside in one of the following ZIP codes: 33142, 33144, 33124, 33146
- Bachelor’s degree in Social Work or related field (NO NURSES)
- Case management & Long term care experience required
- Experience with Medicaid populations and long-term care programs
Position Details
- Work Model: Hybrid (Work from home + field visits)
- Travel: Approximately 75% local travel within Miami-Dade County
- Training: Remote via Microsoft Teams (1–2 weeks)
