HOME HEALTH RN CASE MANAGER | DETAILS AND COMPENSATION
Location: Camden, AL 36726 – 100% On-site, Field work
Coverage Area: Wilcox, Dallas, Marengo, Clarke counties
Pay Rate: $56.14/hour + mileage reimbursement at $0.48/mile
Schedule: Monday–Friday, 8:00 AM – 5:00 PM
On-Call: Every other weekend

HOME HEALTH RN CASE MANAGER | RESPONSIBILITIES
Conduct comprehensive assessments of high-risk patients
Collaborate with primary care providers to implement individualized care plans
Reassess and manage cost-efficient, appropriate levels of care
Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals
Engage specialty resources to ensure optimal patient outcomes
Maintain detailed clinical, functional, and fiscal records throughout care management
Identify opportunities for health promotion and illness prevention
Serve as a patient advocate, ensuring privacy and confidentiality
HOME HEALTH RN CASE MANAGER | REQUIREMENTS
Associate’s or Bachelor’s Degree in Nursing or related field
Current, unrestricted Alabama RN license
Case Management certification
Previous home health experience required
Field case management experience preferred
Knowledge of IV therapy, wound vacs, OASIS, ostomies, Foley catheters, and chest tubes preferred
Ability to analyze complex medical information and make sound clinical decisions
Reliable, professional, and able to manage weekend on-call responsibilities
Mandatory attendance for the first 90 days
This is a contract assignment with potential to hire permanently based on attendance, performance, and business needs.
