Job Description:

As part of our client's dedicated care team, the Home Health Physical Therapy Assistant will be an essential cog in providing empathetic and effective care to our patients residing in Springfield, IL, and nearby locations. The role requires exemplary cooperation in devising and delivering patient care plans, hands-on patient support, and technical aptitude alongside astute time management skills.

Required Qualifications:

- A minimum of an Associate Degree

- At least a year's exposure to TruChart EHR or any other Electronic Medical Record (EMR) system

- Valid Physical Therapy Assistant certification or license recognized in Illinois (IL)

- Valid Regular Driver License (DL) certification for the state of Illinois

- A minimum of one year experience in either a Rehabilitative Setting or Acute Care

Preferred Qualifications:

- Experience of at least one year in Home Health

Key Duties:

- Collaborative inpatient care plan management and delivery

- Direct patient support in Springfield, IL, and neighboring locales

- Timely completion of crucial charting tasks

- Autonomously manage time and schedules

Working Schedule: Traditional Monday - Friday, 8:00 am - 5:00 pm

Travel Requirements: Local
