This Home Health Physical Therapist role offers the opportunity to make a meaningful impact by providing personalized care in patients’ homes. You’ll assess functional needs, develop individualized treatment programs, educate patients, and monitor progress to support improved independence and overall function. New graduates are encouraged to apply and will receive support and mentorship to grow in the role.

Key Responsibilities

• Perform Physical Therapy assessments and develop treatment programs
• Record and report patient's reaction to treatment and condition changes
• Instruct patients and caregivers in therapy programs and device use
• Supervise physical therapy assistant and, when appropriate, the certified nursing assistant
• Prepare reports and participate in staff and Agency development activities

We are looking for compassionate physical therapists with:

• Bachelor’s degree or Master’s degree in Physical Therapy from an accredited college required
• Licensed Physical Therapist issued by the state in which you will be working.
• Minimum of one year healthcare experience but open to new graduates
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations
• A commitment to consistently meet critical deadlines for charting
• The skills needed to self-manage your time and schedule
• Demonstrated experience with tablets, mobile phones and EMR software

PPV position-Rates $60+ depending on type of visit
