Location: Lexington, KY 40509
· Service area includes Lexington-Fayette County & surrounding counties
Pay: $37.30 per hour. Mileage reimbursement provided at $0.43 per mile .
Schedule: Monday – Friday | 8:00 AM – 5:00 PM
· On-call: 1–2 days per week + 1 weekend every 4 weeks
Overview
We are seeking a compassionate and experienced Licensed Practical Nurse (LPN) to provide skilled nursing care in a home health setting . In this role, the LPN will deliver individualized care to patients in their homes, ensuring adherence to care plans while promoting patient dignity, independence, and overall well-being.
Key Responsibilities
- Provide skilled nursing care in accordance with individualized patient care plans.
- Monitor patient conditions, including vital signs, and report any changes or abnormalities.
- Administer medications, feedings, oxygen therapy, and perform treatments such as ostomy care as ordered.
- Support and follow care routines established by PT, OT, and Speech Therapy teams.
- Maintain strict infection control practices and report any potential infections or concerns.
- Accurately document all care provided, patient progress, and communication with care teams.
- Order and manage medical supplies, medications, and equipment as needed.
- Monitor and report incidents, ensuring proper documentation and follow-up care.
- Ensure medical equipment is functioning properly and coordinate repairs as needed.
Productivity Requirement
- Minimum 32 points per week
Required Qualifications
- Active Kentucky Licensed Practical Nurse (LPN) license required
- Home health or community-based care experience
- Strong clinical assessment and documentation skills
- Experience with medication administration, wound care, and patient monitoring
- Reliable transportation for travel within assigned service areas
