Qualifications
LPN License – Current and valid license from the Arizona State Board of Nursing.
Home Health Experience – Strongly preferred.
WellSky (Kinnser) experience preferred.
Driver’s License – A valid Arizona driver’s license.
Reliable Transportation – A dependable personal vehicle with auto insurance.
CPR Certification – Must have current certification.
TB Clearance – A valid TB clearance health card (or the ability to obtain one) meeting federal and state requirements.
Education – Graduation from an accredited Licensed Practical Nurse (LPN) program.
