Home Health Case Manager - Registered Nurse

A-Line Staffing is now hiring a Full-Time, Field Based, Home Health RN Case Manager in Jeffersonville, IN. (COVERING HARRISON, CRAWFORD, ORANGE, PARTIAL FLOYD & WASHINGTON COUNTIES).

Position Overview

We are seeking a dedicated and experienced Registered Nurse (RN) Case Manager to provide comprehensive care coordination for high-risk members. This role focuses on improving patient outcomes through individualized care planning, interdisciplinary collaboration, and proactive intervention.

Home Health Case Manager Compensation

• The pay for this position is $56.00/hr & mileage reimbursement
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with a company match is available for full-time employees with 1 year of service on our eligibility dates

Home Health Case Manager Highlights

• This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs
• The required availability for this position Monday–Friday, 8:00 AM – 5:00 PM with weekend rotation

• Home Health Case Manager Responsibilities

  • Perform comprehensive assessments for high-risk members
  • Develop and implement individualized care plans in collaboration with primary care providers
  • Continuously evaluate and adjust care plans to ensure cost-effective, appropriate levels of care
  • Coordinate care transitions across settings and providers
  • Collaborate with physicians, social workers, discharge planners, and claims professionals
  • Identify and arrange non-medical support services (e.g., housing, transportation) to ensure treatment compliance
  • Engage specialty resources to support optimal patient outcomes
  • Maintain detailed documentation of clinical, functional, and financial outcomes
  • Promote health education, disease prevention, and wellness initiatives
  • Identify risks and intervene to prevent adverse patient outcomes
  • Advocate for patients, ensuring privacy and confidentiality at all times

   Home Health Case Manager Requirements

  • Associate’s or Bachelor’s Degree in Nursing or related field
  • Current, unrestricted Registered Nurse (RN) license in Indiana
  • • Compact license accepted
  • Strong clinical judgment and ability to analyze complex medical information
  • Case Management certification (preferred or required—customize as needed)
