Job Summary:
The PRN Physical Therapist provides high-quality rehabilitative care to residents in a skilled nursing facility on an as-needed basis. This role focuses on evaluating, diagnosing, and treating patients with mobility, functional, and physical impairments to improve independence, safety, and overall quality of life.
Key Responsibilities:
Perform comprehensive physical therapy evaluations and develop individualized treatment plans
Provide skilled therapeutic interventions including exercise, manual therapy, gait training, and functional mobility training
Monitor patient progress and modify treatment plans as appropriate
Document all evaluations, treatments, progress notes, and discharge summaries in accordance with regulatory and facility guidelines
Collaborate with interdisciplinary team members including physicians, occupational therapists, speech therapists, and nursing staff
Educate patients and caregivers on treatment plans, home exercise programs, and safety strategies
Ensure compliance with Medicare, Medicaid, and other payer guidelines
Maintain patient confidentiality and adhere to HIPAA regulations
Participate in care planning meetings as needed
Qualifications:
Doctor of Physical Therapy (DPT) or equivalent from an accredited program
Current state licensure as a Physical Therapist
Current CPR/BLS certification
Previous experience in a skilled nursing facility or geriatric setting preferred
Strong clinical reasoning and decision-making skills
Excellent communication and interpersonal skills
Ability to work independently and adapt to varying patient caseloads
Work Schedule:
PRN (as needed) based on facility census and staffing needs
Flexible scheduling; may include weekdays, weekends, and holidays
Physical Requirements:
Ability to lift, transfer, and assist patients with mobility (up to 50+ lbs with assistance as needed)
Prolonged standing, walking, bending, and reaching
Ability to respond quickly to patient needs and emergencies
Work Environment:
Skilled nursing facility setting
Exposure to patients with acute and chronic medical conditions
Fast-paced, team-oriented environment
Compensation:
Competitive hourly PRN rate
No guaranteed hours; schedule based on facility demand
