Job Description:
We are seeking a compassionate and motivated PRN/Full-Time Occupational Therapist (OT) to join our interdisciplinary team in a skilled nursing facility setting. The ideal candidate is dedicated to helping residents regain independence and improve quality of life through evidence-based occupational therapy interventions.

Responsibilities:

• Evaluate residents and develop individualized treatment plans

• Provide skilled OT services to short-term rehab and long-term care patients

• Document evaluations, progress notes, and discharge summaries in accordance with facility and regulatory standards

• Collaborate with PTs, SLPs, nursing staff, and physicians

• Educate residents and families on therapeutic techniques and safety strategies

• Maintain productivity standards while delivering high-quality patient care

Qualifications:

• Licensed Occupational Therapist (OT) in the state of practice

• Graduate of an accredited Occupational Therapy program

• SNF or geriatric experience preferred (new grads welcome to apply)

• Strong communication and documentation skills

• Ability to work independently and as part of a team
