Field RN Case Manager Compensation
• The pay for this position is $58.14 per hour
• Benefits are available to full-time employees after 90 days of employment
• A 401(k) with company match is available for full-time employees with 1 year of service on eligibility dates
Field RN Case Manager Highlights

Field RN Case Manager Responsibilities
• Perform comprehensive assessments of high-risk members
• Collaborate with primary care providers to implement individualized, comprehensive care plans
• Identify and coordinate cost-effective, appropriate levels of care
• Arrange non-medical support services such as housing and transportation to support treatment compliance
• Coordinate care transitions with physicians, social workers, discharge planners, and claims professionals
• Engage specialty resources as needed to support optimal patient outcomes
• Maintain detailed clinical, functional, and fiscal documentation
• Identify opportunities for health promotion and illness prevention
• Prevent and intervene in adverse patient occurrences
• Act as a patient advocate while maintaining privacy and confidentiality
Field RN Case Manager Requirements
• Associate’s or Bachelor’s degree in Nursing or related field
• Active, unrestricted Registered Nurse (RN) license
• Ability to analyze complex medical information and make sound clinical decisions
• Case Management Certification preferred
